| Study: Use of Management Tools for Chronic Conditions Varies Widely Posted: 16 Dec 2009 06:08 AM PST Use of care management tools such as group visits or patient registries varies widely among primary care physicians whose practices care for patients with asthma, diabetes, congestive heart failure and depression, according to a national study by the Washington-based Center for Studying Health System Change . Three-quarters of physicians said they offer written educational materials, but use of other tools for patient education and improved self-care was much lower, the study found. For example, half of physicians reported using non-physician educators, one-third used nurse managers to coordinate care and one-fifth used group visits. Practice size and setting were strongly related to the likelihood that physicians used care management tools, with solo and smaller group practices least likely to use care management tools, according to the study. HIE Picks Software Platform Posted: 16 Dec 2009 05:54 AM PST The Healthcare Information Xchange of New York (HIXNY) has selected the Cambridge, Mass.-Based InterSystems HealthShare software platform as the core data exchange software to connect healthcare organizations throughout 17 counties in northern New York State that are served by the RHIO. HIXNY is a not-for-profit RHIO composed of a group of organizations including hospitals, physician practices, health plans, healthcare associations, government/education and consumer/patient groups. HIXNY’s members include health plans representing over 400,000 members; major regional hospitals with over 3,000 beds; large physician groups representing a total of 500 physicians; healthcare associations representing 56 hospitals and over 30 health plans in New York State; a university/school of public health; and a consumer advocacy group. |
